National Food Fortification Program in Iran: A Policy Brief

Background and Objective: Micronutrient deficiency is one of the main nutritional challenges in Iran and worldwide, which can lead to growth disorders, reduced productivity, and increased disease burden. Despite the implementation of mandatory fortification programs in Iran, these programs have faced numerous challenges. This policy brief aims to investigate the challenges and barriers to the implementation of the national fortification program and to propose policy solutions for its execution.
Methods: This research was conducted from September 2012 to November 2014 using a mixed-method approach (library-based and qualitative). A search for documents was performed in reputable national and international databases, and out of 78 initial documents, 33 relevant documents were analyzed. Additionally, 7 focus group discussions were held with 15 members of the Scientific Committee on Food Fortification, along with a one-day national conference and a meeting of the National Committee with the participation of all key stakeholders. Qualitative data were analyzed using content analysis.
Results: The analysis of findings indicated that the lack of a codified national policy on fortification, the absence of a systematic program for evaluating the effectiveness of these programs, and weak inter-sectoral coordination were the most important barriers to the implementation and sustainability of fortification programs. To address these problems, the National Food Fortification Program was developed in three sections (Regulations, Administrative and Operational Structure, and Technical Requirements).
Conclusion: Implementation of the National Food Fortification Program requires high-level approval by the Supreme Council for Health, sustainable financing through the Plan and Budget Organization, regular activation of the National Committee, and human resource training. This program can reduce micronutrient deficiencies and improve the health and economic productivity of the country.
